Clinical Overview

Vortioxetine is a novel antidepressant with multimodal activity indicated for major depressive disorder. It combines serotonin reuptake inhibition with direct modulation of multiple serotonin receptor subtypes, potentially providing antidepressant efficacy with cognitive benefits and reduced sexual side effects compared to traditional SSRIs.

Primary Clinical Applications

Vortioxetine is indicated for the treatment of major depressive disorder in adults. It may be particularly beneficial for patients with depression who experience cognitive symptoms, as clinical studies suggest it may improve cognitive function independent of mood improvement. It may also be suitable for patients who experience sexual side effects with other antidepressants.

Mechanism and Clinical Benefits

Vortioxetine inhibits serotonin reuptake while also acting as an agonist at 5-HT1A receptors, partial agonist at 5-HT1B receptors, and antagonist at 5-HT3, 5-HT1D, and 5-HT7 receptors. This multimodal activity may enhance cognitive function, reduce sexual side effects, and provide antidepressant efficacy through multiple pathways.

Clinical Considerations

Vortioxetine has a relatively favorable side effect profile with lower rates of sexual dysfunction, weight gain, and discontinuation syndrome compared to many SSRIs. However, nausea is common, particularly during initiation. The medication has linear pharmacokinetics and reaches steady state in approximately 2 weeks.

Prescribing Information

Dosing & Administration

Major Depressive Disorder – Adults:

  • Initial: 10 mg once daily
  • Range: 5-20 mg once daily
  • Titration: May increase to 20 mg daily as tolerated
  • Maximum: 20 mg once daily

Dose Adjustments:

  • Poor CYP2D6 metabolizers: Maximum 10 mg daily
  • Concurrent strong CYP2D6 inhibitors: Reduce dose by 50%
  • Concurrent strong CYP inducers: Consider increasing dose up to 3x normal (maximum 60 mg daily)

Administration:

  • Take once daily with or without food
  • Same time each day for consistency
  • If dose reduction needed due to tolerability, consider 5 mg daily before discontinuation

Indications

  • Treatment of major depressive disorder in adults

Contraindications

  • Hypersensitivity to vortioxetine
  • Use with MAOIs (within 14 days)
  • Use with linezolid or IV methylene blue

Warnings & Precautions

  • Boxed Warning: Increased risk of suicidal thoughts and behavior in patients under 25 years
  • Serotonin syndrome: Risk with concurrent serotonergic drugs
  • Activation of mania: Monitor in patients with bipolar disorder
  • Discontinuation syndrome: Taper gradually when stopping
  • Abnormal bleeding: Increased risk with anticoagulants
  • Hyponatremia: Particularly in elderly patients

Drug Interactions

  • MAOIs: Contraindicated – minimum 21-day washout from vortioxetine
  • Strong CYP2D6 inhibitors: Reduce vortioxetine dose by 50%
  • Strong CYP inducers: May need to increase vortioxetine dose
  • Serotonergic drugs: Increased risk of serotonin syndrome
  • NSAIDs, aspirin: Increased bleeding risk

Adverse Reactions

Common (≥5%):

  • Nausea (most common, often transient), vomiting, constipation
  • Diarrhea, dry mouth, dizziness, abnormal dreams

Advantages:

  • Lower rates of sexual dysfunction compared to SSRIs
  • Potential cognitive benefits
  • Minimal weight gain

Special Populations

  • CYP2D6 poor metabolizers: Maximum 10 mg daily
  • Hepatic Impairment: No dose adjustment needed for mild-moderate impairment
  • Renal Impairment: No dose adjustment needed
  • Elderly: No specific dose adjustment, monitor for hyponatremia
  • Pregnancy: Category C – use only if benefits outweigh risks
