Modafinil is a wakefulness-promoting agent indicated for excessive sleepiness associated with narcolepsy, obstructive sleep apnea/hypopnea syndrome (as adjunct to standard treatment), and shift work sleep disorder. It provides sustained wakefulness without the jitteriness and rebound effects commonly associated with traditional stimulants.

Primary Clinical Applications

Modafinil is indicated for narcolepsy, obstructive sleep apnea (as adjunct to CPAP), and shift work sleep disorder. It is also used off-label for fatigue associated with multiple sclerosis, depression, and attention deficit disorders, though these uses require careful consideration.

Mechanism and Clinical Benefits

Modafinil’s exact mechanism is not fully understood but involves dopamine reuptake inhibition, histamine activation, and effects on hypocretin/orexin neurons. It promotes wakefulness without significant effects on sleep architecture and has lower abuse potential than traditional stimulants.

Clinical Advantages

Compared to traditional stimulants, modafinil has a lower risk of tolerance, dependence, and cardiovascular side effects. It provides sustained wakefulness for 12-15 hours without significant rebound hypersomnia, making it ideal for shift workers and patients requiring consistent alertness.

Prescribing Information

Dosing & Administration

Narcolepsy and Obstructive Sleep Apnea:

  • Standard dose: 200 mg once daily in morning
  • Range: 200-400 mg daily
  • Administration: With or without food

Shift Work Sleep Disorder:

  • Dose: 200 mg once daily
  • Timing: 1 hour before start of work shift

Elderly:

  • Consider lower doses due to slower elimination
  • Start with 100 mg daily

Hepatic Impairment:

  • Severe impairment: Reduce dose by 50%

Contraindications

  • Known hypersensitivity to modafinil or armodafinil
  • History of left ventricular hypertrophy
  • Mitral valve prolapse with CNS stimulant use

Warnings & Precautions

  • Serious skin reactions: Stevens-Johnson syndrome, TEN, DRESS
  • Psychiatric symptoms: Mania, delusions, hallucinations, suicidal ideation
  • Cardiovascular effects: Monitor blood pressure and heart rate
  • Multi-organ hypersensitivity: Rare but serious reactions
  • Abuse potential: Schedule IV controlled substance

Drug Interactions

  • Oral contraceptives: Reduced effectiveness, use alternative contraception
  • CYP3A4 substrates: May decrease levels (cyclosporine, midazolam)
  • CYP2C19 substrates: May increase levels (phenytoin, diazepam)
  • Warfarin: Monitor INR

Adverse Reactions

Common (≥5%):

  • Headache, nausea, nervousness, rhinitis, diarrhea, back pain

Serious:

  • Serious skin reactions, psychiatric symptoms, cardiovascular events

Special Populations

  • Pregnancy: Category C – use only if benefits outweigh risks
  • Hepatic Impairment: Dose reduction required in severe impairment
  • Elderly: Lower starting dose recommended
  • Renal Impairment: Use with caution, limited data
