Mechanism and Clinical Benefits
Donepezil reversibly inhibits acetylcholinesterase, the enzyme that breaks down acetylcholine in the brain. By increasing acetylcholine levels, it helps compensate for the cholinergic deficits characteristic of Alzheimer’s disease, potentially improving memory, attention, and daily functioning.
Administration and Tolerability
Donepezil is taken once daily at bedtime to minimize side effects. The medication has a long half-life allowing for consistent therapeutic levels. It is generally well-tolerated with primarily gastrointestinal side effects that often diminish with continued use.

Bradycardia and syncope risk
Cholinergic action can produce bradycardia and heart block in predisposed patients. Unexplained falls or syncope in a patient on donepezil should prompt cardiac assessment rather than being attributed to the dementia.
Gastrointestinal effects usually settle
Nausea, diarrhoea, and vomiting are the most common adverse effects and typically diminish with continued use. Bedtime dosing and slower titration both help.
Anticholinergics work directly against it
Co-prescribed anticholinergics, including over-the-counter antihistamines and bladder antimuscarinics, oppose the mechanism of action. This combination is common and often unnoticed.
Caution around anaesthesia and airways disease
May enhance succinylcholine-type neuromuscular blockade, so anaesthetists should be informed. Use caution in asthma and COPD, and in patients with bladder outflow obstruction.
Prescribing Information
Dosing & Administration
Mild to Moderate Alzheimer’s Disease:
- Initial: 5 mg once daily at bedtime
- Titration: May increase to 10 mg daily after 4-6 weeks
- Maintenance: 5-10 mg once daily
Moderate to Severe Alzheimer’s Disease:
- Dose: 10 mg once daily
- High-dose option: 23 mg once daily (after at least 3 months on 10 mg)
Administration:
- Take at bedtime before retiring
- With or without food
- ODT formulation dissolves on tongue without water
