Clinical Overview
Donepezil is a cholinesterase inhibitor indicated for the treatment of dementia of the Alzheimer’s type in mild, moderate, and severe stages. It is the most widely prescribed medication for Alzheimer’s disease and has demonstrated efficacy in improving cognitive function and activities of daily living.
Primary Clinical Applications
Donepezil is indicated for all stages of Alzheimer’s disease, from mild to severe dementia. It may slow cognitive decline and help maintain functional abilities for longer periods. The medication can be used as monotherapy or in combination with memantine for moderate to severe disease.
Mechanism and Clinical Benefits
Donepezil reversibly inhibits acetylcholinesterase, the enzyme that breaks down acetylcholine in the brain. By increasing acetylcholine levels, it helps compensate for the cholinergic deficits characteristic of Alzheimer’s disease, potentially improving memory, attention, and daily functioning.
Administration and Tolerability
Donepezil is taken once daily at bedtime to minimize side effects. The medication has a long half-life allowing for consistent therapeutic levels. It is generally well-tolerated with primarily gastrointestinal side effects that often diminish with continued use.
Prescribing Information
Dosing & Administration
Mild to Moderate Alzheimer’s Disease:
- Initial: 5 mg once daily at bedtime
- Titration: May increase to 10 mg daily after 4-6 weeks
- Maintenance: 5-10 mg once daily
Moderate to Severe Alzheimer’s Disease:
- Dose: 10 mg once daily
- High-dose option: 23 mg once daily (after at least 3 months on 10 mg)
Administration:
- Take at bedtime before retiring
- With or without food
- ODT formulation dissolves on tongue without water
Indications
- Treatment of dementia of the Alzheimer’s type (mild, moderate, and severe)
- May be used in combination with memantine
Contraindications
- Known hypersensitivity to donepezil or piperidine derivatives
- No absolute contraindications
Warnings & Precautions
- Gastrointestinal effects: Monitor for nausea, vomiting, diarrhea
- Cardiac effects: May cause bradycardia, heart block in predisposed patients
- Respiratory effects: Use caution in patients with asthma or COPD
- Seizures: May lower seizure threshold
- Genitourinary effects: May worsen bladder outflow obstruction
- Anesthesia: May enhance neuromuscular blocking effects
Drug Interactions
- Anticholinergics: May reduce donepezil effectiveness
- Cholinergic agonists: May enhance cholinergic effects
- CYP2D6/CYP3A4 inducers: May decrease donepezil levels
- NSAIDs: Increased risk of GI bleeding
Adverse Reactions
Common (≥5%):
- Nausea, diarrhea, insomnia, vomiting, muscle cramps
- Fatigue, anorexia, ecchymoses
Serious:
- Severe bradycardia, syncope, seizures, GI bleeding
Special Populations
- Hepatic Impairment: No dose adjustment needed for mild-moderate impairment
- Renal Impairment: No dose adjustment needed
- Elderly: No specific dose adjustment required
- Pregnancy: Category C – not typically used in pregnant women
